Soma Integrated Psychiatric (All Arizona Wellness PLLC) respects your privacy. This policy explains what information we collect when you visit somaintegratedpsych.com and how we use it.
This policy covers our website only. Your medical records and protected health information (PHI) are governed by our Notice of Privacy Practices, which is provided to every patient at intake and is available in the patient portal, by mail, or at our office on request.
Information you give us
If you use our contact form, we receive the information you type: your name, email address, and message. We use it only to respond to your inquiry. We do not sell it, share it for marketing, or add you to mailing lists without your permission.
Please do not send sensitive health information through the contact form or by standard email. These channels are not encrypted. For anything involving your diagnosis, medications, or clinical care, please use the secure patient portal or call us at 480-382-8284.
Information collected automatically
Our website is hosted by Wix.com. Like most websites, Wix collects standard technical information from visitors, such as browser type, device type, pages visited, and approximate location, using cookies and similar technologies. We use this information only in aggregate to understand how the site is used. You can limit cookies through your browser settings. Links to other services
Our site links to the ChARM Health patient portal (phr.charmtracker.com), which we use as our secure electronic health record. ChARM is HIPAA compliant and operates under a Business Associate Agreement with our practice. Once you are on the portal or any other outside site, that site's own privacy practices apply.
Email and text messages
Standard email and SMS are not fully secure. We use them only for scheduling and administrative communication, never for sensitive mental health or substance use information without your explicit request. Patients can set communication preferences or opt out at any time; see our Electronic Communications Consent or contact the office.
Children's privacy
This website is not directed to children under 13, and we do not knowingly collect personal information from children through it.
Your health information rights
As a patient, you have rights under HIPAA and Arizona law, including the right to access and amend your records and to receive an accounting of disclosures. These rights are described in full in our Notice of Privacy Practices. Substance use disorder treatment records receive additional federal protection under 42 CFR Part 2.
